Permission to View Your NHS Summary Care Record
The NHS Summary Care Record (SCR) is an electronic summary of key clinical information (including medicines, allergies and adverse reactions) about you, sourced from the GP record. It is used by authorised healthcare professionals such as pharmacists and pharmacy technicians who have been trained in SCR usage.

Summary Care Records are created and maintained by GP practices. You can opt-out of having an SCR altogether and you should contact your GP practice who will record this, if this is your preference.

In providing care to you, Drugsmith Pharmacy would like to view your Summary Care Record in order to provide the best possible care for you. Whenever your record is accessed, the details will be recorded should you wish to question this at any point in the future. You can change this permission at any point just by contacting us.
